  • Step 1: Download Order Form

    Click the button below to Download our Google Sheet, input all of your contacts and shipping destinations.

    First things first: after you open the order form using the button above, download the Google Sheet as an XLSX file. To do this, hit File > Download > Microsoft Excel (.xlsx)

    You will now have a fully-editable copy of the file on your local computer.

    Please do not request edit access to the original document. We cannot grant this permission as it will destroy the original template for everyone else.
